Community Agents is a free to access home visiting service, helping people to live independently in their own home. We can help anybody over the age of 18 that is in particular need of a home visit in order to source the support needed. Community Agents are part of the Essex Wellbeing Service.

Whilst we don't provide specific mental health support, there are many areas of your lifestyle that contribute towards poor mental health, such as emotional wellbeing which we do provide support for. This also includes social isolation and weight management.

If you're an Essex resident you can turn to us for help with quitting smoking; managing your weight and/or your child’s; becoming a volunteer; gaining support with social isolation, and making long-term friendships.We'll also cover some of the areas that contribute towards these lifestyle changes, such as alcohol consumption, physical activity, healthy eating, and mental health.Through the delivery of NHS Health Checks, we also help to identify early signs of poor health and identify lifestyle changes required.

This includes making onward referrals and signposting to services; accompanying clients to appointments; and advocating on behalf of clients. Futures Together also provides a Social Inclusion service that supports people with substance/alcohol misuse and/or mental ill health through a range of peer-led, meaningful activities for people to engage in (no criminal justice involvement needed for this).

A range of free weight management services for adults registered with a GP surgery in Essex. Our programmes are designed to promote safe weight loss of between 0.5-1 kg (1-2lb) a week. Introducing steps one week at a time to help build the tools that will enable confident use for continued weight loss. We believe in helping people to #LoseWeightHappily. This consists of positive, motivational and educational solutions to help you lose weight for good. We know losing weight can be difficult, emotional and testing, so we’re here to help.
