Service Detail

The Care Coordination Centre(CCC) is the referral hub for Community Services. Provided by community nurses, Life Limiting nurses in palliative care, case managers, occupational therapists, physiotherapists, speech and language therapists, dieticians, respiratory nurses, Falls Car and Urgent Care Response (UCRT).

Open 8am to 9pm, 7 days a week, including Bank Holidays.

How are services delivered?

The CCC service can be contacted by healthcare professionals, patients and family members for information or advice and support. Available to patients registered with a West Essex GP.

Care is provided in patient’s homes and in care home across west Essex. All referrals are triaged in the CCC to ensure patients receive the appropriate service to meet their needs.
